Who handles information

Crash Review Desk handles information submitted through this website. Privacy questions can be sent to jakub@crashreviewdesk.com.

What the website collects

The crash review form may collect your name, role, work email, carrier name, USDOT number, crash date and state, whether the carrier has the report, and your consent to be contacted. The website and NuaSite form infrastructure process the form so that Crash Review Desk can receive and respond to the request.

If you email us instead, the message may include the same details and anything else you choose to write. The website host may also process ordinary security and server-log information.

What not to send

Do not send a police accident report, photographs, video, crash narrative, signature, driver, witness or medical information, Login.gov or DataQs credentials, MFA codes, recovery codes, FMCSA PINs, authenticated sessions, or payment-card data through the website or ordinary email.

If a case advances, we verify authority and provide a separate secure route for case evidence.

Public carrier data and outreach

We may maintain public carrier and crash facts obtained from FMCSA or other official sources, along with business contact data and a minimal suppression record used to prevent duplicate or unwanted contact. A public crash row is never treated as proof of fault or CPDP eligibility.

Service providers and international access

NuaSite form infrastructure, website hosting, email, security, and professional advisers may process information as needed for their functions. Crash Review Desk may receive information about US carriers from outside the United States. Case evidence uses a separate authorized transfer process.

Retention and choices

Inquiry and correspondence records are retained as needed for service, security, suppression, dispute, and legal purposes. A minimal suppression record may remain after an opt-out so that contact does not restart.

You may ask not to receive further marketing contact, correct information you supplied, ask about information associated with your request, or request deletion subject to verification and legal exceptions. Email jakub@crashreviewdesk.com. Do not send identity documents in the first message.

Cookies and analytics

The website contains no advertising tracker, behavioral profiling, customer account, checkout, or report upload. The form provider and website host may process the technical and security information needed to receive a request and protect the service.
